Required Documents
For all adults 18 and over:
– Legal ID
– Proof of Address
For all children 17 and under
– Birth certificate or immunization record
– If the guardian lives outside of BA or Coweta current proof of enrollment in BA or Union Public Schools will need to be provided.
** Gifts are only for children 15 and under **
